A stage 1 pressure ulcer is characterized by nonblanchable erythema of intact skin. This means that the affected area of skin appears red and does not turn white (blanch) when pressure is applied. The redness indicates that there is inflammation and blood flow is still present, but the skin integrity remains intact, which is crucial for identifying this initial stage of skin breakdown.

In contrast, other signs associated with more advanced stages of pressure ulcers, such as blister formation, visible slough, or necrotic tissue, are not characteristics of a stage 1 ulcer. Blisters indicate superficial damage, while slough and necrotic tissue suggest deeper tissue injury, corresponding to stages 2, 3, or 4 pressure ulcers. Recognizing nonblanchable erythema is essential for early intervention and prevention of further skin damage.
